Prema Narayan was born on April 4, 1955, in Kalimpong, West Bengal, India (some sources also mention Calcutta/Kolkata as her birthplace).

She is the niece of renowned actress Anita Guha, which gave her a family connection to the cinematic world. Narayan began her career as an English teacher trainee in a convent school before transitioning to modeling, inspired by her aunt Anita Guha.

In 1971, she participated in the Femina Miss India contest and was crowned Femina Miss India World 1971. That same year, she represented India at the Queen of the Pacific pageant held in Australia, where she was crowned first runner-up (also described as Queen of the Pacific). Her success in beauty pageants opened doors to the film industry, and she chose Bollywood after offers started coming her way.

She made her film debut in the Bengali hit "Amanush" (1975), for which she won a nomination for the Filmfare Award for Best Supporting Actress. She played the role of "the Other Woman" in Amanush.

Throughout her acting career during the 1970s and 1980s, she worked in more than 65 films, including "Manzelein Aur Bhi Hain" (1973), "Maa Bahen Aur Biwi" (1973), "Umrao Jaan" (1981), "Mangalsutra" (1981), and "Pataal Bhairavi" (1985). She also appeared in Mahesh Bhatt's film with Kabir Bedi and Gulshan Arora as her breakthrough.

She worked predominantly in Hindi cinema as a supporting actress, often getting roles as second leads with song and dance scenes.

She did a few Bengali films and also performed songs in South Indian movies. As a dancer, she sang her way into several hits of the 1970s, including "Botal se ek baat chali" in Ghar, "Machali oh manchali" in Barsaat Ki Ek Raat, and other popular songs. After her career as a leading lady did not make headway, she switched to supporting roles.

Prema Narayan is married to Rajiv, a lawyer, and the couple has two sons. Today, she is recognized as an Indian former actress, model, dancer, and beauty pageant titleholder. At age 67 (as of recent reports), she is described as a happy wife and mother, far removed from show business.
